10Km with a 19db should have got a prety good signal, I think you are much lower than the waveguide and if my memory serves me correctly the terrain is also a lot lower where you are?

Would have to work it our but one thing you may like to try is to downtilt the antenna so as 10 degrees off the top (upward) is pointing at the horizon.
