If your SIM card was a really old pre-merger AT&T Wireless card you might
be able to get a replacement here:

They also sell fairly new AT&T SIM cards for a lot less than AT&T
usually does. If your SIM cards are really old (not 310-410) Cingular
cards, say from California, however, I don't know where you'll get one.
As for the other bit, note that AT&T's Average Revenue Per User (i.e.
per line of service) at $50/month is the lowest among the nationwide
carriers. By this standard someone paying $114/month for 4 lines isn't
really a high spender, they're significantly below average.
