trouble with WUSB54Gv2, Mandriva 2006, ndiswrapper

ndiswrapper loads but the device isn't showing up in the dmesg output and
iwconfig doesn't show it either. ndiswrapper -l shows the driver installed
and hardware is present. Googling hasn't provided an answer and neither has
the ndiswrapper FAQ. lsusb doesn't show the device but Mandrivas UsbView and
lshw sees the device. I've tried uninstalling and re-installing both the
device driver and ndiswrapper. The device has power and it works when booted
to the Windows partition. I'm using the driver that came on the CD for the
WUSB54Gv2. Does anyone have a solution? (And yes, I followed the
installation wiki even though I'm quite proficient with ndiswrapper).

Mandriva 2006, stock kernel 2.6.12, ndiswrapper 1.19rc2.

If you google WUSB54Gv2 it seems many people on many distros have had to
deal with some issue related to that card.
