Default CPW chequeback - 4th or 5th bill?

Can anyone tell me what bill I send off for the chequeback? They want
bill 4, but I have had other CS people saying send bill 5 because the
"1st" one doesn't count.
Can anyone clarify/confirm?

I have had exactly the same nonsense from CPW - either it's the 4th
bill or it's not - how can they say the first bill 'doesn't count'?!

As they allow me 60 days to send in the bill, I'm sending them months 4
*and* 5 and I'll still have at least 30 days to spare.

Are you sending them together or seperateky?
