Why don't you want a monthly contract?
That seems to me to be the answer to your problems.

Because it would allow a child to run up an unlimited bill. I go along
with the suggestion to let them manage their own credit.

Surely any auto top-up system would also allow a child to run-up
an equally big bill?

Good way is to let them take responsibility for the balance - knowing they
will be grounded if they want to go out with a balance of zero on their
phone

That's what I meant. I did not say an automatic top-up system, I said let
them manage their own credit which is exactly what you suggest.
